Sac Collab for Our Community

The Sacramento County Collaborative (Sac Collab) is a community partnership with a vision to improve health and wellness in Sacramento neighborhoods.

The Sac Collab started in June 2020 as a response to COVID-19. Sacramento County Public Health and The Center at Sierra Health Foundation partnered with community-based organizations throughout the county to support neighborhood residents. Our partner organizations know the communities they serve. They connect with and support individuals and families with a focus on their wellness and equity.

Earlier in the pandemic, the Sac Collab helped the community through COVID-19 Case Investigators, Contact Tracers and Business Navigators. Resource Coordinators and Vaccine Ambassadors helped Sacramento County residents get healthy and stay healthy.
